THE chief investigator into a Cypriot airliner crash outside Athens that killed all 121 people on board said yesterday that human error was chiefly to blame for the accident.

It was the first time that Akrivos Tsolakis clearly indicated human error was to blame for the August 14 crash of the Helios Airways flight from Larnaca to Athens, Greece's worst-ever air disaster.
